For your visit:

2. The front door will be locked when you arrive. Please call and we will let you in or give your order curbside.

3. Only one adult is allowed per appointment. One parent may accompany a minor for a child’s appointment.

4. We will be asking everyone screening questions recent travelling and your current health. You will be required to sign a form with your answers.

5. All patients will have their temperature taken.

6. Please enter our office wearing your mask. If you do not wear a mask, you will be asked to reschedule your appointment

7. We will be providing extra hand sanitizer. Our staff wears protective masks or face shields during your exam.

8. We completely sanitized our exam room after each appointment.
